Our project aims to provide essential respite care and home support services for seniors in our low-income community who are living with Alzheimer's disease and dementia. The initiative will employ certified nursing assistants (CNAs) to offer in-home care, alleviating the burden on family caregivers while promoting disease prevention and treatment, particularly for those with chronic neurodegenerative conditions. Our services will not only enhance the quality of life for seniors but will also support caregivers, who often face emotional, physical, and financial strain.
The need for this service is urgent in our community. With an aging population and limited access to healthcare resources, many families struggle to provide appropriate care for their loved ones, particularly for those with complex conditions like Alzheimer's and dementia. This project will fill a significant gap in both disease prevention and community economic development by providing affordable, high-quality care for those most in need.
Identified Needs
Lack of Access to Quality Care: The local population is economically disadvantaged, and many families do not have the financial means to access professional in-home care services. Seniors with Alzheimer's and dementia require specialized care, which is often unaffordable for low-income households.
Caregiver Burden: Family members, often elderly themselves or working multiple jobs, are struggling to provide consistent care for their loved ones with Alzheimer's or dementia. This can lead to burnout, increased stress, and neglect of both the caregivers' and the seniors' needs.
Growing Senior Population: The county is seeing an increase in its senior population, with a significant number of individuals facing Alzheimer's and dementia-related symptoms. This demographic shift amplifies the demand for care services and places additional pressure on caregivers and healthcare providers.
Health Inequities: Seniors in our community face health disparities due to socioeconomic challenges, including inadequate access to medical care, nutritious food, and mental health resources. These factors worsen the progression of Alzheimer's and dementia, leading to higher rates of hospitalization and institutionalization.
How the Project Will Meet These Needs
This project will provide critical respite care and home support to seniors with Alzheimer's and dementia, focusing on the following areas:
In-Home Respite Care: Certified nursing assistants (CNAs) will provide specialized care in the homes of seniors, offering assistance with activities of daily living (ADLs) such as bathing, dressing, feeding, and mobility. This support will give family caregivers a much-needed break, reducing their stress and preventing caregiver burnout.
Alzheimer's & Dementia-Specific Support: The CNAs will receive specialized training in caring for individuals with Alzheimer's and dementia, including managing behavioral symptoms, promoting cognitive function, and ensuring a safe environment. This care will be tailored to each individual's needs, helping slow disease progression and improving quality of life.
Caregiver Education & Support: The project will include training for family caregivers, providing them with skills and resources to better manage their loved ones' care. This will include information on the disease, techniques for handling challenging behaviors, and ways to support emotional and mental well-being.
Community Economic Development: By employing CNAs from the local community, the project will create jobs and stimulate local economic development. The training and certification of CNAs will also improve the professional skills within the community, contributing to long-term economic resilience.
Project Funds
The funds from this grant will be allocated to the following areas:
Salaries and Stipends for CNAs: A significant portion of the funds will be used to hire certified nursing assistants (CNAs) to provide in-home care. This includes wages, training, and certification costs, ensuring that CNAs are equipped with the necessary skills to care for individuals with Alzheimer's and dementia.
Training & Education Programs: Funds will be allocated for ongoing training for CNAs, including specialized education on Alzheimer's and dementia care, as well as caregiver support programs. This will ensure that both the CNAs and family caregivers are equipped with the knowledge and tools they need to provide the best possible care.
Respite Care Supplies: We will purchase necessary supplies to support in-home care, including mobility aids, hygiene products, medical equipment, and safety modifications for homes. These items are essential for ensuring a safe and supportive environment for seniors living with Alzheimer's and dementia.
Outreach and Community Engagement: Funds will be used to raise awareness of the program and ensure that the families who need it most are aware of the services. This will include outreach efforts in local clinics, senior centers, and community organizations.
Administrative and Operational Costs: To ensure the smooth functioning of the program, funds will be allocated for program management, scheduling, and communication tools to coordinate care and monitor the progress of the project.
Project Timeline
The project will run over the course of one year, with funds being allocated annually to continue the services. The first six months will focus on staff recruitment, training, and establishing care protocols. In the second half of the year, the program will expand to serve more families, with ongoing monitoring and evaluation to ensure the quality of care.
Addendum to Project Description (added after the project was signed)
Budget:
Satellite Club has established a budget for Respite Care of up to $500 per person to be used for the Home Health Care provider. In the past they would contact us with person in need, and we would approve them for funds. The person might not use all the $500, mabye just needed home care for rehabilitation.
Our Budget is still $500 per person that in need of not only home health care but medical necessities.
Club Involvement:
We have worked very hard to engage Home Health Care businesses in the area for Respite Care and Home Health Care in general. We involved Eastern Wyoming Community College's nursing program trying to find a way to involve CNA's with training and income. We contacted several Home Heatlh Care providers in Scottsbluff area. Compass Home Health Care was the only one that was interested in serving Goshen County, WY.
Compass Home Heatlh Care in Scottsbluff, NE has stepped up and they now service Goshen County.
They have developed a referral form and HIPPA authorization form. They also will contribute an additional 25% per case.
Advertising/Marketing
Next month we will start marketing our Rotary Respite Care program.

We have a local contact at the Torrington Telegram newspaper that will do an article on us.
There is a local Alz/Dementia Support Group that we can contact to get the word out.
Local radio does a program called " What Happening in the Valley". We go on and talk about what the Sat. group is doing. In this area the local radio station reaches a lot of Goshen County residents.
